The Chase Brexton Columbia Center is your primary care home. When you become a Healthy Howard Health plan member, you are assigned a primary care doctor who will provide your primary care at this facility. You can make a primary care appointment by calling 410-884-7831.

As a member, you are guaranteed up to six (6) primary care visits per year without an additional fee. As well as these six visits, women enrolled in the plan also receive one annual gynecological exam without an additional fee.

You can make more than six visits with your doctor at the Chase Brexton Columbia Center for an additional fee. Most patients see their primary care doctor two to three times per year. 

The Healthy Howard Care Coordinator is also located at the Chase Brexton Columbia Center. Each time you see your primary care doctor, you will also meet with your Care Coordinator. During your visit, the Care Coordinator can:

• provide information about health services available to you through the Healthy Howard Access Plan;
• help you find reduced prices for prescription drugs;
• and help you make any follow-up appointments with specialists within our network if your doctor makes a referral.
